How to Use Uplifted Lead Capture Forms

Usually, when a Prospect is interested in your school, you add them to Kicksite manually. Using a Lead Capture Form (LCF) allows leads to fill out their own information, which then creates a Kicksite profile.

What are Lead Capture Forms?

LCFs are used in two ways; on their own, and in Landing Pages. When used by themselves, they’re a simple way to collect contact information, and can be embedded on your website or linked to with a QR code or short link.

Landing Pages are like mini websites where leads can sign up for class, purchase inventory items, sign agreements, and more. Learn how to create Landing Pages here.

How to Create a Lead Capture Form

  1. Click the Marketing Tools section on the sidebar (represented by a target), then click Lead Capture Forms.
  2. Click the Add New button in the upper right.
  3. Enter a title for the LCF. This is for internal use only, and must be unique.
  4. (Optional) Change Advanced Settings.
  5. Add or edit the Form Content.
    • Email and phone number fields are included and required by default. 
    • Make a field optional by toggling Required to off.
    • Rearrange fields by clicking and dragging on the gripper dots.
    • Remove a field by clicking the trash can. If you change your mind, click the Restore icon to add it back.
  6. Click Add. The Form is ready to embed, share, or include in Landing Pages.

Custom Form Content

To collect information other than birthdate, address, sex, and contact information, you can add Custom Fields. There are two types of custom field:

  • Custom Text Field: An open text box
  • Custom Dropdown: Leads will select an option from a dropdown menu. Specify the choices in the Dropdown Options field. Enter a comma between each option.

For all Custom Fields, you can choose where on the person’s profile you want that information to appear. You can enter as:

  • Comment – Will appear in the Comments module of their profile
  • Pinned comment – Will appear in the profile’s left sidebar and the Comments module
  • Guardian information – Will appear as Guardian 1 or 2’s name or phone number

If you are using both a Participant and Guardian Lead Capture Form on a Landing Page, do not select Guardian 1 as an Enter As option on the Participant form. This will conflict with the information entered by the Guardian on the Landing Page.

Advanced Settings

Click Advanced Settings for these options:

  • Customize colors shown on the Form.
    • When you add an LCF to a Landing Page, the Landing Page’s colors override the ones on the LCF.
    • To enter a hex code, click on the color label.
    • The eyedropper tool can color pick from somewhere else on your screen.
    • The “shadow” color affects small details like: divider lines, section borders, and the box behind the SMS Opt-In message.
  • Turn on email and/or text notifications for submissions. Formerly known as Quick Connect.
    • You can select multiple admin or employee email addresses.
  • To create Contact profiles instead of Prospects or Students, turn off Create Participant Profile on Submission.
  • To use a source other than the Lead Capture Form’s name, turn on Use Custom Source.

Archiving Lead Capture Forms

When you no longer want to accept submissions on a Lead Capture Form, archive it. Like other things Archived in Kicksite, this keeps a record of the Form and all its submissions. You can still view it in the Archived tab of the Lead Capture Form list.

If someone tries to fill out an Archived LCF, they will see a page telling them it is no longer accepting submissions.

To Archive a Lead Capture Form, follow these steps:

  1. Click the Marketing Tools section on the sidebar (represented by a target), then click Lead Capture Forms.
  2. Find the Form you want to archive, then click on its title. This takes you to its Details page.
  3. Next to the title is the Active status bubble. Click on the bubble, then select Archived from the dropdown.
  4. In the popup window, confirm you want to Archive the Lead Capture Form.
    • You must remove the LCF from any Landing Pages before you can archive it.
    • Each connected Landing Page will be listed. Swap those to use a different Lead Capture Form, then return to Archive the original.

To reactivate a Lead Capture Form, turn the Archived status bubble to Active. Customers can then access it and fill it out.

How to Export Lead Capture Form Submissions

If you want a list of all the submissions to a Form, go to the Lead Capture Form’s details page, then click Export.

The submissions will be combined in a .csv file, which you can open in Excel, Numbers, or Google Sheets.

Frequently asked questions

What happens to my Landing Pages if I update the attached Lead Capture Form?

Editing a Lead Capture Form will also update any Landing Pages using that Form. However, past submissions will not be affected. All the Landing Pages that an LCF is attached to are listed in the Included section on the list.

Why would I make a separate Guardian Lead Capture Form?

You can select two Lead Capture Forms for a Landing Page: a participant (Student or Prospect) one, and a guardian one. Using both forms can be useful – for example, you may need the participant’s allergy information, but not their parent’s. 

If you don’t select a Guardian lead capture form, everyone will fill out the same fields. If the person signing up is 18 or older, they will have all the participant fields, plus the contact information required of a guardian (they count as their own guardian).

When a Guardian is entered, the Prospect or Student created in Kicksite will display the Guardian’s information in the Guardian 1 field. The Guardian will not have a separate Contact profile.

When Family Sign-Ups are released, the Guardian Lead Capture Form will allow creating a Primary person who isn’t a Student. Learn more about Families here.
